Annual TB draw (2013) This is for ARUP employees that are in need of getting their TB blood draw. This is only open from October till December
Cronic Illnesses (Initial Visit) Help Manage the following: Establishing primary care, new medications (Rx), Mental Health
Follow-Up Please make follow up visits with the same Medical provider that did the evaulation. This will create consistency and better patient care.
This visit includes perscription refills (Rx)
General Clinic Visit (Other) Schedule here if you don't know what category to schedule under. Examples: Consults, Wart removals, ect.
Orthopedic If an X-ray is needed you will be charged to go outside the clinic for the X-ray.
PHP 2013 (Personal Health Profile) Come in fasting for 12 hours (drink water to keep hydrated). We will draw a lipid and fasting sugar panel in addition to conducting biometric testing. ****MUST COMPLETE BEFORE SEPTEMBER 30, 2013***
Physicals Types of Physicals: Scouts, School, Missionary, Sport, DOT, PAP smears, General
Procedure Procedures can ONLY be scheduled after an evaluation has been preformed. Make sure to make appointment with the SAME medical provider that did the evaluation.
Procedures include: Mole removals, Casting, Skin tags, etc.
Well Child Checks Any age from couple weeks old to 18yrs of age. Please don't schedule appointment before the needed age mark. This will postpone possible needed vaccines.
