AppointmentQuest online partner enrollment is a set of forms organized in a manner of easy to use wizard that will guide you through the entire partner account creation process. The following information is required to complete enrollment process:
• Business name, business address and contact phone number
• Valid credit card for signup fee processing (not required for Affiliate accounts)
• Partner account administrator name, contact information and desired username
