You can set your appointment quest account to deliver email appointment reminders. However, email is an imperfect method of communication. Emails can be lost, sent to junk, or simply vanish into cyber space. Therefore, please don't entirely rely on these reminders. You are responsible for your appointment whether or not you receive an email reminder.

Payment is due at the time of service. We take cash, checks and credit cards.

While we do not bill insurance directly, upon request my office will provide you with an attending physician statement which is a detailed receipt that you can submit to your insurance company directly or use for income tax purposes. This statement has all of the appropriate codes and information necessary for your insurance company to reimburse you, should your plan cover our services.
