  First Visit  

 Please arrive 20 minutes early to fill out paperwork for your first visit, or download intake forms from website and bring to your appointment. We have a 24 hour cancellation policy with a $15 fee for late cancellations or no shows. Cash and check only.

 Please LOGIN before making an appointment. We have a 24 hour cancellation policy, otherwise there is a $15 fee. Our online appointment system automatically blocks schedule changes within 24 hours of any given appointment.
