Marie L. Eloi-Stiven, MD reserves the right to deny providing a service to a client based on her medical judgment. If a client is not a candidate for a particular procedure Spa Doc, LLC and Dr. Eloi-Stiven will make a reasonable effort to provide alternative treatments and/or professionals to accommodate a client’s need.

Please provide at least 48 hour notification if you plan to cancel you appointment. If you cancel, our on-line appointment scheduling system may not allow you to reschedule. Therefore, call 718-459-8677 to reschedule your appointment. Thank you.

Payment must be provided at the time of service. Medial Insurance is not accepted for cosmetic procedures and currently Spa Doc, LLC does not accept insurance. Spa Doc, LLC reserves the right to request a deposit prior to the procedure. There is no refund for cosmetic fillers (Radiesse) or Botox after the procedure has been performed due to the nature of the procedure.
