1. First Visit | | Please arrive 20 min. early, or download intake forms from website and bring to appt. The 15 minute time-slot refers to initial interview time, NOT appt. length which varies from 20 minutes to 2 hours depending upon patient's needs/preference. | |
| 2. Return Visit | | We require 24 hours notice for any cancellations, otherwise you will be responsible for paying your usual treatment fee ($15 minimum). Our online appointment system automatically blocks schedule changes within 24 hours of any given appointment. | |
