 New patient evaluation. 
 Make Appointment for NEW PATIENT - $200 

  Office Visit - EXTENDED - $150  

 Established Patients.This office visit is for medical conditions that require more time with your physician. 
 Make Appointment for Office Visit - EXTENDED - $150 

  Office Visit - STANDARD - $90  

 Established Patients. Standard office visit for acute and chronic medical conditions. 
 Make Appointment for Office Visit - STANDARD - $90 

  Physical Exam - $150  

 Established Patients. Office visit includes urine test and EKG. Other tests as needed. 
 Make Appointment for Physical Exam - $150 

  SAME DAY or NEXT DAY - $90  

 Appointment for patients who have already been seen/established. Same day medical evaluation for infections or other acute issues. Scheduling available within 24 to 48 hours of appointment time. 
 Make Appointment for SAME DAY or NEXT DAY - $90 

  Well Woman Exam - $150  

 Established Patients.This visit includes Pap Smear and urine test. Other tsts as needed. There is an additional commercial lab fee for the Pap Smear if you do not have insurance to cover the test.
