| Physical Exam/$150 | | This visit includes EKG, Hemoccult stool test, Snellen vision exam, and urine test. | |
| SCHOOL, SPORTS or SCOUT Physical/$100 | | This visit includes a physical exam, urine test and completion of required paperwork. | |
| Well Woman Exam/$150 | | This visit includes a physical exam with Pap Smear, Hemoccult, and urine test. There is an additional commercial lab fee for the Pap Smear if you do not have insurance to cover the test. | |
