  First Visit  

 On your first visit you will need to fill out a health history form. Please arrive 10 minutes early or download the form from our website and complete it at home. We will spend a few minutes going over your paperwork before beginning the treatment.

 Please note that we have a 24 hour cancellation policy. We charge $15 for missed or late canceled appointments.
