  A. Initial Consultation  

 Please bring the completed Health History Questionnaire and Medication/Supplement log. These forms are available at www.HealAndRenew.com or regular mail. Call 973-921-0348 for assistance

  B. Report of Findings with Acupuncture  

 The ROF is scheduled after your initial consult. Dr. Pelechaty will review your clinical findings, recommend a treatment plan, dispense remedies, and review life-style guidelines. He will also conduct acupuncture if indicated.

  C. Acupuncture Treatment  

 Acupuncture appointments are 30 minutes. Please have a light snack before acupuncture.

  D. Follow-up Health Consults  

 Health Consults are performed as part of your treatment plan. Appointments are 30 minutes. Please bring questions!
