1 hour- New Patient Visit (Adult) | | This is a 1 hour visit for adults over 18 (Under 18, use Well Child) or for children with significant medical histories. Please complete the appropriate online paperwork as detailed on my website and be aware that I charge an annual fee ($160/patient). | |

1 hour- Well Child Exam | | Select this visit for a check-up for any new or established patient under the age of 18. Please print and fill-out the age appropriate history form PRIOR to your visit (see the website). Please note that I charge an annual fee ($160/patient). | |

30 min visit: Urgent/Short Follow-up | | Select this appointment for all Short Follow-ups or Urgent visits. If you need to be seen urgently and no visits are available in the schedule, page me, Dr. Hirsch at 360.480.0353, and you will be seen in the next 24 hours. | |

45 min Follow-up Visit/Annual Exam | | Select this 45 minute visit for any routine medical follow up visits for adults and children having 2 or more issues that need to be addressed at the visit. | |

Allergy/Acupuncture Visit - 1 hour | | On the website, Acupuncture patients should complete the "Getting to Know You" Survey prior to the appointment and Allergy patients should print and read the instructions under Allergy under the "Patient Portal" tab. | |
