1. New Patient Visit (Adult) - 1 hour | | This is a 1 hour visit for adults over 18 (Under 18, use Well Child) or for children with significant medical histories. Please complete the appropriate online paperwork as detailed on my website and be aware that I charge an annual fee ($160/patient). | |

2. Simple Follow-Up Visit - 30 min | | Select this appointment for all routine and simple follow-up appointments. This includes going over laboratory results and other simple medical issues. | |

3. Same Day/Urgent visit - 15 mins | | Select this appointment for all Same day or Urgent visits. If you need to be seen urgently and no visits are available in the schedule, page me, Dr. Hirsch at 360.480.0353, and you will be seen in the next 24 hours. | |

4. Complex Follow-up Visit - 45 min | | Select this 45 minute visit for any medical follow up visits for adults and children having 2 or more medical issues or any complex issues that need to be addressed at the visit. | |

5. Well Child Exam - 1 hour | | Select this visit for a check-up for any new or established patient under the age of 18. Please print and fill-out the age appropriate history form PRIOR to your visit (see the website). Please note that I charge an annual fee ($160/patient). | |

6. Acupuncture Visit - 1 hour | | On the website, Acupuncture patients should complete the "Getting to Know You" Survey prior to the appointment. | |

7. Allergy Testing - 1 hour | | This appointment is for patients who have already been evaluated by the practice and determined to require allergy testing. Please print and read the instructions under Allergy on the "Patient Portal" tab at http://www.doctorevan.com. | |

8. Injections (IM) - 5 mins | | Select this appointment for all Vitamin B12 and Testosterone shots. All IV injections appointments should utilize a 30 min appointment slot. | |
