New Patient Appointments
We currently have a waiting list for Dr Hirsch, but have availability for Jan Soloy. Please download, print and fill out the Medical Questionaire form from our website: http://doctorevan.com/history.pdf. Please make sure we have the form in our office 48 hours prior to your appointment for Jan's review.
Appointments
If you are an established patient and would like to schedule a future appointment, please do this online. Please reschedule your own appointments using your personal user name and password in Appointment Quest. Our office provides an automated email appointment reminder two business days prior to your scheduled appointment as a courtesy to you.
Billing
Please make sure all your insurance information is up to date. Always bring your insurance card and photo id to every visit. It really helps with the administration, filing and processing of each visit. A demographic information update is required every six months. Please have your insurance information and photo id handy so we can process your information in a timely manner.
Please notify us as soon as possible if you are unable to make an appointment as we reserve the right to charge for visits canceled with less than 24 hours notice. 3 No show visits in a 12 month period an we may ask you to leave the practice.
Instructions for Scheduling and Completion of Lab Work
You are welcome to contact a laboratory of your choice to schedule to have your labs drawn. It is important that you bring your lab requisition form with you. If you need to cancel or reschedule your upcoming lab review appointment, please do so online.
You should notify our office if you have lost or need a copy of the requisition form as soon as possible. We can email the requisition form to you. After completion of your lab visit, please make sure you have an appointment scheduled to review the results with your provider.

We have recently updated our options when calling the office. Please listen carefully and select the option which best serves your needs. If you have questions please email us. Please do not call the office and leave messages if it is not urgent, as these calls will not be responded to.
