New Patient Forms may be downloaded at www.nhacupuncture.com, e-mailed to you, sent by mail or set out at the front desk for you. If you cannot fill your forms out before the day of your appointment, please arrive 20 minutes early to complete them. Treatment will not begin until the forms are completed and signed.
